His previous psychiatric history included hallucinogen-induced psychosis requiring inpatient treatment a year ago, associated with persecutory thoughts and vivid hallucinations.
Separating Fact from Fiction: Exploring the Truth Behind CBD and Hallucinations
Hallucinations are a common adverse effect of THC and other hallucinogens, but these effects are rarely described in the peer-reviewed literature following cannabis exposure. The present case describes perceptual alterations and dissociative symptoms that rated highly on the Intensity, Somaesthesia, and Perception subscales of the Hallucinogen Rating Scale (HRS) but with relatively low ratings on the Affect and Cognition subscales compared to HRS ratings obtained from the participant after controlled laboratory studies of other hallucinogen drugs. It is suggested that this experience may be a unique type of psychosis that warrants further clinical investigation. The patient’s interdisciplinary team recommended discontinuation of the CBD product, and he reported that his tremors and vivid hallucinations slowly decreased over time. He was re-prescribed Lewy body dementia medications, carbidopa-levodopa and rivastigmine, and his tremors improved.
